About The Jeremy Bosi Memorial Fund
The Jeremy Bosi Memorial Fund was established in his memory to help those families who otherwise would not be able to participate in the game we all love due to hardship.
Jeremy Bosi #32 played hockey for PHSHA all 4 years, always gave 110%, and had great enthusiasm. He was a hockey coach's dream. It has been said that if you ever watched him play in a game, you would remember it. Tragically at the age of 20, on July 14, 2002, Jeremy was killed in a car accident. Plainfield High School Hockey honors his memory whenever we step on the ice.
To apply for special assistance from the Bosi Fund, families must submit an application outlining the reasons they have been impacted and what they are seeking assistance for. Applications will be reviewed by the Plainfield High School Hockey Board of Directors and are evaluated on a case-by-case basis. A family must be in good standing up until the point of request with Plainfield High School Hockey and be willing to donate volunteer time and services to be eligible for funding. Once a family receives funding from the Bosi fund, they are ineligible for subsequent requests.

To apply for the Bosi Memorial Fund assistance program, please click and download the application on the right.
NOTE: Applicants must have been with the club for a minimum of 1 year, be in good standing at the time of application, and agree to donate service hours for the duration of assistance.
Applications can be submitted to any board member for consideration.
